The Geriatric Care Management Certification course is designed for those individuals who are seeking to become credentialed by the International Commission on Health Care Certification (ICHCC) as a Certified Geriatric Care Manager (CGCM™), including Rehabilitation Services Providers, Long Term Care Professionals, Case Managers, Social Workers and Elder Law / Legal Professionals.
The Certified Geriatric Care Manager (CGCM™) credential is dedicated to the credentialing of professionals who provide services to elderly individuals with health and or disability issues. This credential assesses one's knowledge of care planning and care management.
This course is comprised of four fully online modules, and learning activities in each module include assigned readings, short online videos, knowledge check questions, and case study reviews. The course is designed to be completed in 40 contact hours, and the learning objectives are as follows:
Course participants have the option to attend monthly live online chat sessions hosted by the course instructor on rotating course topics.
Module 1: Overview of GCM Client Assessment & Life Care Planning
Module 2: Ethical, Legal, and Financial Issues in Geriatric Care Management
Module 3: Developing the Geriatric Life Care Plan & Care Monitoring
Module 4: Working in Different GCM Settings & Private Practice
Elizabeth Nanni – OTR, is a licensed Occupational Therapist and Geriatric Care Manager with over 30 years of experience. She is a graduate of the National Center for Rehabilitation in Caracas Venezuela. She has worked for more than 25 years as an OT in the United States providing services to individuals with disabilities and home bound patients. She has extensive experience working with geriatric clients including providing home health Occupational Therapy, private Geriatric Life Care Planning / Management, Home Safety Assessment and ongoing Care Monitoring.
Upon successful completion of the online course, participants should apply for credentialing through the International Commission on Health Care Certification, and complete an application to complete the competency exam. Click here.
